  4. Complete applications (full payment and necessary documentation) will be approved and emailed within 5 working days.
  5. By submitting this application, the applicant agrees that any withdrawal or cancellation request made in writing will be subject to an administration fee of 10% of the total fees paid (non-refundable) and approval by the Management of GS1 Malaysia.

Important Notes: 1. For membership fee breakdown, kindly refer to the Standard Membership fee breakdown, Small Block Membership fee breakdown, Additional Membership fee breakdown and GLN Membership fee breakdown. 2. Standard Membership provides up to 1,000 barcode numbers. 3. Small Block Membership is intended for cottage/micro industry, promotional, festive-run, seasonal-run, limited-run, or voucher products (up to 10 barcodes). Subject to review. 4. Additional Membership offers an extra licence with 1,000 barcode numbers and requires an active GS1 membership. 5. GLN Membership provides 10 GLN numbers for the identification of legal entities, addressable locations (digital and physical, static and mobile) and assets. 5. The security deposit is equivalent to one and a half years' annual fee. 6. Verified by GS1 & GAC Support & Services (VbG & GACSS) is an optional package for managing trusted product and brand data. It is maintained separately from your GS1 licence subscription. 7. VbG & GACSS for Standard Membership: RM 580.00; VbG & GACSS for Small Block Membership: RM 380.00. 8. Barcode Number Generation Service (optional): generates all barcode numbers in PDF format, not applicable for GLN Membership. 9. Barcode Certification Program (optional): validates, verifies and decodes GS1 barcodes, to ensure readability, not applicable for GLN Membership.

I have read the Terms and Conditions governing GS1 Malaysia subscribers to the GS1 System and on behalf of my company agree to abide by said Terms and Conditions. We hereby agree to provide GS1 Malaysia with full access to, and allow GS1 Malaysia the use of and to share all company and product information provided by our company as GS1 Malaysia deems necessary as a trusted source of product information for retailers, international buyers, trading partners and any other marketing and promotion activities.
The Verified by GS1 & GAC Support & Services (VbG & GACSS) is an optional package providing access to GS1 Malaysia Services, Platforms and Capacity Building Sessions. It will automatically renew at least once a year, unless under promotion. If I do not wish to continue to renew the VbG & GACSS, I must inform GS1 Malaysia a minimum of 3 months before the next annual billing cycle, otherwise, the package will continue to be renewed and charges will be billed to my company.
I declare that I am a manufacturer, trader or distributor of medical devices and I will be using the GS1 Standards & Keys for the creation and assignment of Unique Device Identifiers (UDI) according to the US FDA Rule on Unique Device Identification System and that I will keep the information up to date.
